In the case of transgenders, it is simply another dysphoria. Anorexics see starving bodies as still fat. Some with dysphoria see their limbs as not their own and want to have them removed. There are now reports of trans-disabled, like the woman in the UK who had herself blinded because she wanted to be be blind.
The difference is that transgenderism is the only mental illness where the delusion is accommodated per order of the government - let him use women’s spaces to feel like a woman, call him her even though he still has a penis (fewer than a quarter of the men have the male plumbing removed though nearly all grow breasts).
