In the ER we occasionally see transgender patients. Usually it is fairly obvious that a “woman” is really a “gurl”. Ranging from bad drag to stuffed bras, to stubble on the face, to a deep voice, it’s usually not hard to tell what genotype the person was born as.
Anyway, occasionally we are stumped. Sometimes you are just not sure. Sadly its rarely where you confuse a man for a beautiful woman. Men simply look better as men – unless you have extremely fair skin, slight features, hormones, and surgery. So usually in the cases where one is not sure, it’s the “either she’s a really unattractive woman or a very feminine man”.
Recently we had a case where people were taking bets. The name “she” used was androgynous (not Pat but something similar) so we were stumped. The voice was very feminine but deep. Luckily the person was complaining of abdominal pain so there was a medical reason to peek underneath the gown to confirm. Sadly, I lost that bet. Luckily no Crying Game scenario though!

I saw my doctor yesterday for my annual physical. He has a person working in his office that until he grew a small mustache and beard you were not sure if it was a he or a she. Last year, before the mustache, I was observing this short round person with boobs, a sweet feminine face and a shaved head and wondered which it was. Last year I asked it’s name and it was Chris, still no way to tell, it might as well have been Pat.
There’s no polite way to ask whether someone has a willy.
