The fundamental flaw with ACOs is the Big Brother approach of controlling costs by dictating how physicians may practice. Doctors may decide they'd rather be 'accountable' to their ACO paymasters rather than patients.
A product of last year's massive health-care reform law, ACOs represent the latest government attempt to control escalating health costs. Set to take effect next January, they will use financial carrots and sticks to "encourage" doctors and hospitals to merge into large Accountable Care groups, where they'll give care to Medicare patients according to government "cost effectiveness" guidelines. These ACOs would be rewarded for spending less than government budget targets (or penalized for exceeding them). Government would monitor ACO performance with mandatory electronic medical record systems.
Suppose you saw your doctor for a severe headache and he said, "You don't need an MRI scan. Just take two aspirin and call me in the morning." Could you be sure he was giving you his best medical advice â without being biased by the recent memo from the ACO administrator warning that he might lose his "efficiency bonus" for ordering too many tests?

This may date me, but I bought the book that detailed the proposed Hillarycare, it was the worst horrorstory that fortunately didn’t become reality... until now.
Back when Slick Willie tried to shove Hillarycare down our throats, we didn’t want it then. And we don’t want socialized medicine now. Fortunately, in 1993, Congress recognized they actually work for us.
So this great socialized medicide experiment was foist upon all the stupid Massholes who elected Myth Romney as their Governor.
And the ugly sequel of Romneycare is Obamacare.
Medicare, Medi-whatever, Stimulus and Affordable Housng have already broken the bank. Why do they need more bank-breakers?

For example a new Medicare Commission, exempt from judicial review, will unilaterally write rules about utilization and pricing of medical devices and drugs often needed by surgeons. Many regulatory bodies will be overarching in their abilities to select and direct medical subjects concerning Tri-Care, employer group policies, Veterans Administration, Medicare, Medicaid, etc.
Diligent govt personnel could not fit all of O'Care's new laws boards, commissions, mandates, and other elements on this chart. So they created bundles of bureaucracy.
Seven collective symbols respectively represent clusters of four loan repayment and forgiveness programs, four other new regulatory programs, 17 insurance mandates, 19 special-interest provisions, 22 other new bureaucracies, 26 other new demonstration and pilot programs, and 59 other new grant programs.....

and yet - they’ve done nothing about tort reform.
So - when the doctors miss important problems as they cut back on testing -the malpractice suits will skyrocket.

We have Tenncare in TN...it is on it’s 3rd round of downsizing, as it is bankrupting the state.
Both Ryan’s plan and 0’care are identical for the first 10 years. No FIXING OF THE 40+ years of donor palm greasing. No going after the $60 Bil in fraud and waste. No Patient input, only lobbyist (former congress critters) and donor professionals input.
I guess I’ll be a more fortunate senior.
I feel so sad for my remaining kin.
