I hadn’t seen this certification of live birth! You are correct there is missing data. I also love the fact that a stamp is there right on the front of the document as it should be.
Who in their right mind places a stamp on the back of a document? That is unless they don’t want to certify what is written on the front.

I’d have to look to be sure of the details, but in the HDOH Administrative Rules it mentions some amendments that are to be written on the back of the certificate, versus those that must be noted on the face of the certificate. I’m wondering if the stamp would be put on the back of the certificate if something was required to be noted on the back of the certificate.
I’ll have to look up those details.
