I was always alert, and just folded the diaper back up, as sometimes, they can pee in thier own faces!

I've already had experience with this because of two of my sister's baby boys of the past.
Air makes them go, so many times you want to hold it a minute because of you change them fast, they will wet the new diaper all at once.

Rule:
Open diaper with hand out or large trash can lid ready.
Leave diaper in place and be ready for flood #2 before diaper change.
Have plenty of diapers handy.
