Well missing her period is the first clue, but not always the definitive clue. But only the woman knows about that — not her friends, lovers, or parents.
In 1960 the doctor didn’t even want to see you until you had missed 2 periods. Then, they’d poke and probe you, do a little test that only the doctor could do (no at home tests in 1960), disappear into the back room for a while and come out smiling to say, “Congratulations Mrs. So & So, you are sitting on a nest!”
At least that’s how my doctor informed me that my oldest was on the way. It was up to me to decide when to announce it to others.
As for your second premise — there are a lot of people walking around who are the result of a first time encounter.
