Yep, the application for a new issue of the form was withdrawn. It wasn’t created. the form you are looking at is the Feb 1990 form. Pretty hard for it to have been signed in 1981, eh?

I don’t see a Feb any year that ends in 0

So it is unclear what is taking place. They don’t show the actual form in anything I have found so far. So when does the date change?

But there was a form in 1980. Doesn’t anyone have an image of it?
