Americans are stuck with the illogical and immature notion that healthcare is some kind of unlimited, infinite good available to everyone, an entitlement to everyone merely because of their existence as individuals.
Like anything in this world, health care must be rationed. Supply is not infinite.
We also labor under a simplistic, and false, paradigm which has never existed in reality of two extremes of "market freedom and efficiency" on one side, and "socialist central planning and equity" on the other. We've been pushing towards the bloated bureaucratic socialist side for many decades now.
Even In supposed extreme laissez faire periods, churches, nuns and communities ran collective hospitals and networks to take care of the poor and their local neighbors. Conversely, even in the most marxist regimes, health-care is still rationed by financial ability (paying bribes), one's political class, or connections to power.
Like most government functions in America, health care needs radical decentralization. Let localities decide what is needed, and how they themselves will fund it. They will come up with their own ideal local blend of freedom/capitalism/collectivism/socialism.

I feel the same way about schooling.
