so why do these accidents happen often?

Often poor crossing design. The road way frequently has an abrupt “hump” at the tracks. Some tractor trailer combinations have low ground clearance and get hung up.

Yes, they shouldn’t be using those particular roads and crossings. But, it often difficult to know in advance of the hazard.

Many railroad crossings seem to have been used for registering mortars.
