I doubt that there is a state where this sort of fraud is not happening on a large scale. That is a big reason our national debt is so large.

Some of the conservative Southern states resisted Medicaid expansion on the basis that the state’s share of the costs would spiral out of control. Yes, there is fraud everywhere, but the massive fraud will be in states with Democrat governments, lots of immigrants, and numerous welfare recipients. Triage investigative resources on those criteria.
I’ve never understood why Medicare and Medicaid fraud could not be stopped quickly by a few algorithms tied to id numbers for the billers and the doctors who authorized the “services”. Billing for over some modest amount should have triggered cut-offs pending thorough review. The programs were intended as scams by design; the House, Senate, and federal bureaucrats had to be complicit.
