The solution is to let rural communities declare themselves as a tribe, get said recognition and a tribal health center. My Dad's hometown has an indigenous tribe with a nice clinic which also treats the rural white people since there aren't enough tribal members to keep them busy.

That is a nice set-up, also because it’s nice to have some traditional Native touches to medicine on top of conventional.
