The history of that sort of event is mostly that of people standing on the accelerator, who insist afterwards they were standing on the brake pedal. See the Audi “unintended acceleration” hit piece by CBS 60 Minutes, and the aftermath, for example.

Exactly. According to a series of tests done by Car & Driver on a variety of cars, applying full throttle while braking adds about 10% to the stopping distance. Cars simply do not have enough power to overcome properly functioning brakes.
