The answer to those questions is : When a Pope is installed but not selected by the Holy Spirit. Or, to put it simply: When a Church is infiltrated.

Re Q's drop about the Pope as the Bishop on the chessboard, I just saw an article about Mel Gibson's latest movie about Christ, Mary Magdelen. I recall when he was making The Passion of The Christ (2004),  he or his father was said to be "sedevacantist." 

The term for a Catholic who believes the Pope is not presently of God's choosing is "sedevacantist." This belief holds that the papal seat is vacant due to the current pope's perceived heresies.
