There was no excuse for that. It was criminally negligent behavior (at the least) by those who authorized it and those who did it even though it was "authorized protocol". These were medically trained people. They should have known better - or should have.

Prosecutions. We need civil and criminal prosecutions - all the way up and down the chain of command so it won't happen again.
