The ship is a Krivak-class frigate. The weapon that malfunctioned is an SS-N-14 Silex SSM/ASW missile. The warhead of the Silex is either an ASW torpedo or nuclear depth charge. It was the Soviet version of the US Navy ASROC. In this case the warhead was a torpedo.

And that is an example of why the Ukrainians are kicking the Russian Navy's butt in the Black Sea.
