I think President Trump was truly stunned at how bad Biden was.
It remains hard to believe that any political party would place their obviously senile nominee in that situation.
There are many explanations that have been offered—some more credible than others—but it remains a “one off” event that will probably never be repeated.
I believe he handled it beautifully. He could have really attacked him hard like we all know he can, but he knew he would be seen as mean for attacking a senile old man.

We still don’t know the full story of the attempted assassination in Butler, PA, a couple of weeks later, and may never know. If it wasn’t a “lone nut” like the government and the media would like us to believe, the planning for that may have started right after Biden did so badly in the debate. It was only after Trump miraculously escaped death that Biden dropped out of the race.
