Merely by making them all pass a few tough exams prior to allowing them to join the team. This is something "I'm OK, you're OK" morons wouldn't consider.
Thought for sure it would involve a shove out the airlock.
Seriously though, I suspect they pump the person full of tranquilizers before they hurt themselves or the other astronauts.
