Quentin Tarantino had a good interview on responsibility on a movie set. Said 90% is the armorer and 10% the actor. He talked about how dangerous blanks are so if an actor is expected to shoot three times in a scene, the gun is loaded with three blanks. If only two are shot, it is the actors responsibility to make the director, armorer etc. to draw attention that only two fired.

On the Rust set, live rounds were introduced and loaded into the prop gun by the inexperienced and careless armorer. Contrary to industry rules and practices, Baldwin failed to check the gun personally for safety. He then clowned around and fired the gun toward the camera crew, killing the cinematographer.
