If I recall correctly, an executive at the publishing house accepted responsibility for having failed to adequately fact check the claimed place of birth. This would have been many years earlier, in an earlier stage of her career. I do NOT recall that she — or anyone else — ever accepted responsibility for originating the false claim.

Totally agree. At that point in time, Maobama was still an unknown and no publisher is going to waste one second of time doing serious research into the biography of an author. Ask any author that question of ‘where did the bio come from that was in the liner notes’?
This is akin to speaking at a conference and half an hour before going to the podium, the chair of the conference comes up and says “Hey, can you give me a few lines of a bio for yourself? I’ve got the job of introducing you.....”
