I think that Biden’s eyes were dilated by drugs and he was instinctively trying to avoid the harsh studio lights.

At one point he was looking at the wrong camera. They tried to help him by switching camera’s but he still was looking in the wrong direction.

Looking away from Trump so as not to be distracted.
Trying to concentrate on the voice in his ear but because of his age and dementia he didn’t have the presence of mind or training to pull it off smoothly.
Many tv hosts have to deal with show producers constantly feeding them info while they’re live on stage, but it takes training.
Biden’s toast.
Huge pupils, obviously on drug enhancements, but just added to his confusion.

On a serious note: I think the podiums were angled toward each other and the moderators were directly in front. When he looked to his left he was looking at the moderators.
