The survivors will always wonder if there wasn't something they should have seen, said, or done to try to prevent it.

No, not necessarily. Depression can drive someone to places he or she can't possibly comprehend. It's a torturing NUMBNESS to it all, so much so that the temptation becomes to do anything to feel sensation if only for a fleeting second.
