I completed the survey and told them that I would not be returning to their restaurants. And the waitresses tip was reduced because I as the customer was being required to do her job.

That is actually more common than you might imagine. Even in situations like this in a "captured" market. People find ways to avoid fees and taxes, very easily actually and they do.

They can do it, but don’t expect a 20% tip when you do.
