Control your eating and get out and move.
Unless you have a genuine metabolic disorder, that will be all you need.

Many people and there is a societal socioeconomic aspect simply don’t care anymore and feel little pressure to stay somewhat normal body weight

It’s nothing more than caloric discipline
No disease makes anyone 2-3 times normal body weight
