Is there more to the 2013 cdc page screengrab, such as the dates the CDC says the polio shot was given?
The synopsis above it says 1955 to 1963...

I found the whole page, see below. Interestingly even all the entries in the Wayback Machine are giving the CDC "Oops" page. It's been scrubbed.
