Am not an expert on that field or subject, but once upon a time it was a narrow field of practice, treatment was heavy on the psychiatric side. Lots of counseling. And if staff agreed that it was a legit step to take, hormones would be allowed, with monitoring of effects along with continued counseling. Surgery was seen as a last, radical step only for certain circumstances, with continued care afterwards that was still heavy on the mental counseling. That sort of treatment strategy was planned out by experts the way it was done for the sort of reasons we now see exploding all over the place. They evidently knew what sort of dynamite they were working with and operated under concerns for the liability not only unto themselves and their practice but society at large as well.
