I have been in the NYC Supreme Court when they assigned cases. A clerk takes a case walks over to a bingo style hand cranked cage and spins it, reaches in, grabs a token indicating the judge, puts it together with the case.

The person that put the names in the hat put the same name on each paper for the mother to pick from.

The weak link is the clerk. No one checks to see what the token actually says.
