Pilots have heart issues and emergencies all the time also.

My friend who was a non commercial pilot was always examined heart wise after he developed diabetes. Did not lose his license till he developed proven cardiac disease. My guess is that commercial pilots have to be fit to fly. Do not remember so many reports of pilots having cardiac arrest or issues while flying. Or soccer players....or regular folk dying while exercising.
