Usages of the terms "y'all" and "all y'all," as from the mouth of my ancient 8th grade teacher, Miz Battle:
- "Y'all gonna be the death'a me."
- "I want all y'all to hush rett nayow."
- "Y'all give me an Excedrin headache."
- "Now all ya'll need to clean up this mess retnayow."
- "Boy, what is wrong witchoo? Y'all gonna put me in my grave." (Pretty sure she was referring to all boys and not just this one, but could be wrong.)
I'm pretty sure it's all plural... but I've never diagrammed these sentences.

You description of your 8th grade teacher is definitely North Carolina accurate. Too many from the north in Wake County (Raleigh) to hear “y’all or all y’all” much but here in Johnston County it is quite common.
