If the same weapon was loaded later and fired using a blank conversion cylinder (and noone had checked to ensure that the barrel was not obstructed) the lodged projectile would then be propelled out w/lethal force.

He fired one round and the slug went straight through the cinematographer’s chest and then hit the director in shoulder who was standing behind her. It was one bullet that killed her and wounded the guy standing behind her.
