The gun in Brandon's case had been used previously for close-ups. It was a revolver. To make it look like it was really loaded, dummy loads were used (bullet in a casing with no primer or powder). However, the dummy round was actually a squib (primer and bullet, no powder). During the first scene, the trigger was pulled and the squib ignited. It was strong enough to propel the bullet into the barrel but not not all the way out. It wasn't a fragment it was the whole bullet. The same gun was then later used with blanks that had a powder charge sufficient to force the stuck bullet out the barrel.

I've had a squib. Thankfully it wasn't strong enough to cycle the action on my 1911. Knew something was wrong as soon as it fired. Took some time to get the bullet out of the barrel with a wooden dowel. I quadruple check my reloads to this day because of that.

It doesn’t seem to me that Baldwin was all that traumatized (maybe he was). In his statement he said that “MY heart breaks for ...”, but ASAIK never apologized to the woman’s family. It’s always about him.
