I will do the same and at Liberty Counsel, they say to do both medical and religious in the same submission. However, I’m having trouble finding medical letter samples. Have you seen any that you could post or send to me? It’s a fine line in claiming exemption and not giving them more details than they are entitled to have (HIPPA).
I haven’t seen a medical exemption letter, but this site has a form letter with questions that could be tweaked to include in a medical exemption letter.
