I followed that thread - it was also a prophylactic dose, not the treatment dose (day 1 and day 3)
iMATH protocol is .2mg/kg for prophylactic use, to .4mg/kg post exposure (day 1 and 3).
For treatment it is .4 to .6/kg per day until recovered.
So, according to my calculations- the treatment recommended for a 63kg person (140 lbs) is almost 26mg to 38mg. Check my math.

The doctor here who says he has not seen it work only prescribed 12mg (irregardless of weight), for a handful of friends/family, for treatment

Math is fine. 200 micrograms per kilogram means about 12 milligrams for a 130 lb. person. In the case of treatment, if using 400 to 600, you get numbers right in line with what you posted.
(p.s., I’m old enough to remember when doctors were allowed to use the drugs they thought best and actually prescribed the doses to take)
