At least your agency added the “Important Reminders”. That was missing from the email I got.
Fortunately I’m a remote employee so unless I have to travel to a government building I won’t have to get the jab or get tested. And I can retire, which gives me some leverage.

My take on the form-
Is it untruthful even if someone says on the form “decline to respond”?
That’s not saying one way or the other; they’re not saying “yes I got the shot” when they didn’t, and “no I didn’t get it” when they did.
Hang in there.
