No signature at shot time. They just look up the appointment in their laptop and hand you the VAERS hotline/website paper. There might have been some checkbox when I signed up online with some fine print, but I don't remember anything like that. It was the default Virginia DOH website that you get when you look it up.

Well hopefully it worked and you stay healthy. My wife and I have auto immune disorders and with the meds we take we can not take the vaccine. And I would not take it anyways. But that’s just my choice. We wore masks and social distanced but caught the covid anyways. Stuffy nose and was really tired for 5 days. That was it.
