I have no problem with insults based on looks, as long as they’re reasonably on target. “Pencil neck” nailed Schiff perfectly, and was immediately meme-able and funny!
OTOH, Saying Fauci’s daughter looks like a man is an insult that’s feeble and not on target, as far as I’m concerned, because to me she really doesn’t. That particular bunch of insults just made me cringe.
