You can download your own off the Internet as a PDF and print it out on a piece of card stock if you want to be really authentic, or just print one out look up pictures of some to write down some legitimate lot numbers of vaccines, add some random medical scribble for the location, take a picture and be done with it
For an extra touch, you can use clear plastic packing tape on both sides of your fake card to make it look laminated.
