Here's my personal theory on this:
PDJT is too busy with matters of utmost and earth-shaking importance to be "doing" the account, which is quite an active, busy one. And, as others have mentioned, the sense of humor seems to differ from his - to be more juvenile, raunchy and in-your-face. I'm guessing Don Jr. is responsible for this. Perhaps others, such as Scavino, may be involved. Maybe even Barron. LOL
But yes, I agree, all or most of the content gets run by PDJT for approval. I can picture PDJT even laughing embarrassedly at some of it, and shrugging his shoulders, thinking, "well, since this isn't 'really' me (wink wink), okay, let it post!" The very essence of plausible deniability.
As to who is actually running the account, my guess would be that 'Catturd' fellow, and he does the graphics and videos as well, IMO, since there is so much prominent collaboration work there, and he is fluent in such things already. For example:
