Yes I have my CDC card with two stickers on it. The first one has my date time, manufacturer, batch number, vial number, my name and texas DL number. The second sticker has all the same plus a QR code in the end of it. I would bank on that’s the link to the state database that when scanned will bring up my record of vax. They scanned my email QR code for entry, my DL both sides and the vial of the vax before they pulled my dose out of it so they have everything they need to send to the CDC for a national database I would imagine other states are doing the same process.
