Hydrogen peroxide and viral infections: A literature review with research hypothesis definition in relation to the current covid-19 pandemic

https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7262503/

Conclusions
There are no randomized controlled trials or clinical observational studies concerning the curative or preventive effect of hydrogen peroxide against viral infections. However, the literature from in-vitro immunological studies clearly points out that the application of hydrogen peroxide on the epithelial cells of nose, throat and mouth may well be extremely effective against viruses, including coronaviruses. We strongly encourage the rapid development of randomized controlled trials in both SARS-CoV-2 positive and negative subjects to study the effects that we have hypothesized. Those trials would be inexpensive to implement and compatible with other ongoing trials. Positive results would lead to remarkable global health gain with extremely limited costs. As a starting dose to test, we propose two puffs (about 0.28 ml) of 1.5% H2O2 nasal spray into each nostril two times daily combined with a mouth wash and gargling for 1 min with a 3% H2O2 solution two times daily.

Lowering the transmission and spread of human coronavirus

Our results suggest that several nasal/sinus and oral rinses had potent virucidal properties and could have the potential to inactivate HCoV and decrease viral load in vivo.

Studies of chronic rhinosinusitis have shown the safe use of 1% baby shampoo formulations as a nasal rinse,21-25 but there is no literature to date that evaluates its use against HCoV or other viruses. Our study shows that a 1% baby shampoo solution was effective at inactivating HCoV in a time‐dependent manner. The dilute rinse was able to reduce the amount of infectious virus by close to 99% after a contact time of 1 min and greater than 99.9% after a contact time of 2 min. With a contact time of 30 s 1% baby shampoo showed variable results ranging from less than 90% reduction in infectious virus to up toward a 99.9% reduction. Overall the results show a clear time‐dependent decrease of infectious virus. In contrast, a commonly used saline rinse formulation (Neti‐Pot) had no effect on infectious viral count in our study.

Listerine Antiseptic is an alcohol‐based eucalyptol, menthol, methyl salicylate, and thymol formulation that historically has claimed numerous antimicrobial properties. It currently lists only a claim to kill germs that cause bad breath. Our tests show that it is highly effective at inactivating HCoV in solution (Table 3). Even at the lowest contact time of 30 s it inactivated greater than 99.99% of HCoV. Interestingly, other related products (Listerine Ultra, Equate Antiseptic, and CVS Antiseptic Mouth Wash), while showing substantial reductions, were not as efficient as Listerine Antiseptic (Table 3). These three products were unable to show a reduction of greater than 99.99% with 30‐s contact time. Equate required 2‐min contact time to show a greater 99.99% reduction.

Rhinologists, doctors who specialize in the treatment of diseases of the nose and sinuses, have been using baby shampoo in nasal rinses for years. Compared to rinsing with salt water, baby shampoo does a better job of eliminating viruses and bacteria which can cause post-surgical infection. That’s because the cleansing agents in the shampoo can dissolve the protective membranes of certain bacteria and viruses, thereby inactivating them. However, until recently, there was no evidence that baby shampoo may be effective against Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) which causes Covid.

Baby shampoo may prevent Covid
Researchers at Penn State College of Medicine tested this hypothesis in the lab using solutions containing a strain of human coronavirus that is genetically similar to SARS-CoV-2. The solutions were then treated with various agents, including Johnson and Johnson’s baby shampoo. They then placed the treated solutions in contact with cultured human cells and measured growth of the virus. The results, published in the September 2020 Journal of Medical Virology, show that a 1% solution of baby shampoo deactivated 99.9% of this coronavirus after a two minute period of exposure. ....

snip..Surprisingly, baby shampoo is creating a buzz as a possible way to wash away sinus symptoms. Experts at Baylor College of Medicine are advising many patients to use baby shampoo for sinus infections by adding just a bit to saline nasal irrigations. Saline nasal irrigations are the technique some people use to flush out irritants and mucus from the sinuses.

Research has shown that adding baby shampoo might be more effective than the standard irrigation rinse, featuring just water and salt. A 2013 study from the University of Pennsylvania showed that 60 percent of patients significantly improved after using the concoction for twice-daily irrigations over four weeks.

...Nasal Irrigation Steps

The basic steps for a nasal irrigation are:

Draw the irrigation liquid into the irrigation device of your choice. Many pharmacies carry a variety of types.
Tilt your head down over the sink or bathtub, and rotate left.
Squeeze a few ounces (your doctor can tell you how much) of liquid into your right (top) nostril. As you breathe normally through your mouth, the liquid should flow out your left nostril in a few seconds. Throughout the process, adjust your head if needed so the liquid doesn’t go into your throat or ears.
Repeat the process with the opposite side.
Blow your nose gently to help keep liquid out of your ears.
Check with your doctor before trying nasal irrigation. Each patient’s sinus symptoms are unique, so they require a unique treatment program.
