Look at the neck and also head size disproportionate. Also, the shirts are different between the two pics.
The first picture looks like George Clooney and his wife. The second looks like Epstein.
They images are not altered. I ran through Photo Forensics. Two different days in each image. Clooney also wearing a different shirt.
