A comma always goes after the State name.
Where the hell did YOU go to school? I know you wouldn’t have done well on a New York State Regents exam in the ‘60s.

Another one that drives me crazy is in dates. I see a comma between a month and a year. (Investigation Discovery channel does this ALL the time.) You only use a comma when there is a day date between the month and year.
