thanx. I never dived for that. Cursory results:

I was kinda wrong about the Congo... Kivu Congo was one of the first to get the "fast-tracked" Ebola vaccine under "compassionate use".
Since then, and recently, the Congo declared an end to the 10th Ebola outbreak as the 11th outbreak has begun.( or is the 10th not really over ?)

Phase-II was done in Liberia in 2015. They were ready for Phase-III and they didn't have enough test cases. they expanded to Sierra Leone, and Guinea.
No mention of any casualties.

Side-effects stopped some of the testings for a while.

Other common side-effects were pain at the site of injection, myalgia, and fatigue."

I could find no mention of casualties or permanent impairment because of the vaccine
