The BBC is one of the few halfway objective news sources left and their style is to refer to heads of state as “Mr.” or similar honorific in addition/instead of their formal title. Having heard this usage for decades I’ve always taken it as a matter of style, not as a put-down. Typical usage might be “In a meeting between the US President and the German chancellor, Mr. Trump said...”.
