Myopathy or muscle injury/disease is a known side effect of chloroquine and hydroxychloroquine. HCQ is safer and better tolerated because of its shorter half life.

“Early clinical features of CQ- and HCQ-induced myopathy
include slowly progressive weakness, which begins with the
proximal limb muscles. The initial symptoms are typically
mild, but the painless proximal weakness in the lower extremities can become more intense over time and can spread
to arms and even the face.”
