Yeah, I went to the original document, too, to confirm that is what was there. As you said, you typed it correctly. And I didn’t see the “dosage adjustment” either.

I assume it’s actually written by this doctor. Actually it’s a very informative protocol document he posted. Unfortunately cut-and-paste makes it post weirdly on FR otherwise I would have posted it all.
