The WHO approved (possibly even suggested) the 1957 testing of an experimental polio vaccine in the Belgian Congo which was cultured on Chimpanzee organs and resulted in the transmission of Simian Immunodeficiency Virus (SIV, then unknown) to to humans in the huge (more than 1 million dosed subjects) trial.

In some of the humans, which included many thousands of infants and newborns, the SIV mutated into HIV-1. This was the medical "Accident" which created AIDS.
