I received mine yesterday. I have a question though, I’m the only one in my household since my husband just died, but for some reason I received 2 forms, both addressed to me. I haven’t opened them yet, but am sure one is a duplicate.

Question is, what do I do with the duplicate. Can’t throw it away, not sure if I should just return it. That leaves me with one option. Guess I’ll try to find an #800 for the returned address on the envelope. Just curious if anyone else received more than one.

Open both..the second is only a follow-up. One will give you a code to use on line. Fill one out if sending it in.
