Back in the Stone Age (been a long day and I'm feelin' especially elderly tonight) it was commonly held that 85 percent of aircraft accidents were due to Pilot Error.
Even with those fancy new computers in the birds these days, I don't expect that percentage has changed a bit.
Flying's a tough business for daydreamers.
If the pilot happens to die, it is likely closer to 100% in the event of total aircraft loss or any deaths (other than crew). In the military $10K damage is a class C, and unless it is a bird the pilot will likely eat that. But that is why we get the big money.
But there are no old bold pilots.
