The FBI overstated Steele's importance to the FISA court and didn't follow the Wood's procedures by getting those statements approved by Steele's handler.
The Crossfire Hurricane hid information from the Department officials signing off on the FIRST FISA application, information they had in their possession at the time the application was filed.
The FBI omitted information they had from another government agency that Carter Page was the opposite of a Russian asset and instead had notified that other agency of Russian intelligence officers contacts with him. There's no way they'd get a FISA while including this info.
The FBI failed to notify the FISA court that Christopher Steele himself had told them that one of his key sources was a "boaster" and "may engage in some embellishment"
The FBI falsely told the FISA court that Christopher Steele was not the source of the Yahoo News article, and only shared his information with the FBI and Fusion GPS, even though the Woods File contradicted that by noting he informed the State Department as well.

Shem..cont.Multiple factors effected Horowitz ability to determine what all Comey knew about the falsification of information in the FISA applications and omissions of key details because they weren't able to question Comey and didn't have a record of what was discussed during his briefings.
After the FBI got the first FISA on Page they finally decided to ask Steele if he was a source for media articles, and Steele admitted being a source for the Mother Jones article and was fake fired by the FBI, who continued to use his information through the Ohr backchannel.
The FBI wanted to use Steele's reporting in the "interagency" review of Russian interference and the CIA pushed back. This was in December 2016 for the ICA report so Trump had already won the election. They ended up using Steeles information in an appendix to the ICA.
The FBI was still trying to verify the Steele dossier in November and December of 2016. Comey is such a freaking weasel, he told Trump in January 2017 he should be careful about asking him to investigate the dossier's claims. Tell me again this was never about Trump himself.
The FBI's Validation Management Unit in early 2017 informed Crossfire Hurricane that Steele's past reporting was only "minimally corroborated" which disputed what they told the FISA court. Also they weren't able to corroborate any of his dossier but didn't inform CH of that.
In January of 2017, the FBI interviewed the primary sub-source for some of Steele's statements used in the FISA application and found serious issues and contradictory information from what Steele had reported to them.
By September 2017 the FBI was unable to corroborate most of the Steele dossier's claims. Most of what they were able to verify was already public information at the time he wrote it. They also found his main allegations were inconsistent with their own information they uncovered.
There's an entire paragraph redacted after that. There are minimal redactions so far but this seems like a big one. DECLASSIFY THE IG FISA REPORT!
Now we're at the information the FBI intentionally left out of two or more FISA renewals, like the fact that Steele's Primary Sub-Source for most of the accusations against Page directly contradicted Steele when they spoke with him. Kind of a big deal huh?
And now we get to the altered email from Clinesmith and it is DAMNING. Before the final FISA renewal on Page the other agency specifically reminded the FBI that Page was a source for them, and Clinesmith edited it to say he was NOT a source for them.
The FBI knew Page was a source for another agency against Russian intelligence and repeatedly hid it from the FISA court, even after being asked by other officials and REMINDED by the other agency. That alone would have destroyed their entire case and they knew it.
The FBI also failed to inform the FISA court that not only was it not true that Steele's past work was corroborated or used in criminal tries, that they were given information that Steele had a history of poor judgement and not validating his reporting.
