There's a concrete dome in Rome . . . from Roman times. I don't think they do much, if any maintenance on it. It has a central occulus open to the sky. So it gets weather. It's one of the strongest concrete structures on the planet, IIRC.

IIRC, the bridges require lot os painting maintenance.

The church next to the Leaning Tower of Pisa has an oculus for rain water to come through. According to our guide people had to be baptised there before being able to go into the sanctuary next door.
